For a Guardianship or Conservatorship Inventory, we will need asset values as of the date of your appointment as a Guardian or Conservator.   We will need copies of:

  • Courts letter with required format or packet provided to you
  • Letters of Guardianship or Conservatorship
  • Financial statements 
  • Titles to personal property
  • Deeds for real estate property 
  • Real Estate Tax bill(s)
  • Appraised values of collections
  • Statements of mortgages
  • Statements of vehicle loans
  • Life insurance policies
  • Funeral Trust (if any)

Annual or Final Accountings

For Guardianship or Conservatorship Annual Accountings or Final Accountings, we will need copies of:

  • Court Order for payment of Guardian/Conservator fees
  • List of sources of income
  • Financial statements for reporting period
  • Checkbook register detailing all income/expense transactions
  • Receipts, bills or canceled checks for money spent over $500 (Required)
  • All documents pertaining to the sale of any asset (Required)
  • All documents pertaining to any large purchases (Required)
  • All documents of closed accounts listed in previous accounting
